Tracking and monitoring

 
·       Whole school tracking by assessment coordinator liaising with SENCo.
·       SEN pupils tracked separately liasing with assessment coordinator. Produces a
     termly overview and triggers an action plan.
·       Very focused on individual pupils.
·       Discussion at pupil progress meetings.
·       Termly review of provision maps.
·       Constant dialogue taking place with inclusion team, class teacher, parent, child and
     any specialist.
·       Assessment of pupil to gain support for any formal testing
·       Weekly discussion at Inclusion Meetings.
